EyeconX™ Privacy Policy

This Privacy Policy is designed to inform eyecare Professionals and Patients about EyeconX™'s practices regarding collection, use, and disclosure of information that may be provided via this site. Please be sure to read this entire Privacy Policy carefully before registering with, utilizing or submitting information using EyeconX™.

We believe that in order to earn Patient confidence, it is essential to utilize personal information responsibly. This Privacy Policy is designed to tell you about our practices regarding collection, use and disclosure of information that you may provide via this site.

Privacy Policy

To address privacy concerns, EyeconX™ has actively pursued the best practices and standards for handling personal data. The collection and use of aggregate information is conducted responsibly and respects each Patient's privacy preferences. EyeconX™ has implemented guidelines for handing personal information. Our system was designed to protect personally identifiable information collected in our database. All the personal data collected about the Patient is only used internally by the Patient's own eyecare Professional for the purpose of distributing information according to the Patient's and eyecare Professional's selected preferences. Information, whether it is purely educational content or promotional material, can only be sent to the Patient if the Patient has opted-in to receive information from their eyecare Professional. This opt-in can be found within the 'Meye Profile' section of the Patient website.

What Information Does EyeconX™ Collect?

EyeconX™ has been designed to only disclose Patient personal information to facilitate the Patient's use of the services and for eyecare Professionals to supply a higher quality of eye health care and personalized health information to Patients (e.g. through the Patient's personalized website, called 'Meye Homepage') by providing them with newsletters and/or relevant educational information via the Internet. EyeconX™ will collect certain information about the Patient and maintain and store it on behalf of the eyecare Professional. The eyecare Professional and his/her staff will provide information about the Patient to EyeconX™ for purposes of assisting him/her in offering personalized eye health information to the Patient throughout the normal course of clinic operations. The personal information we receive and maintain about the Patient will be used to either facilitate the Patient's use of the services or to assist the eyecare Professional in providing eye heath services and relevant information to the Patient. EyeconX™ registered eyecare Professionals have the ability to provide appointment reminders, product reorders and relevant eyecare information to registered Patients. Patients and eyecare Professionals are allowed to change their preferences on their site at their own discretion without input or communication from EyeconX™ through their 'Meye Profile' page. Eyecare Professionals also have the ability to revise Patient profiles and are the sole constituents to update Patient diagnostic information.

As described in the next section, our company is unable to identify or access any Patient information and hence, we will not and cannot disclose personally identifiable information about the Patient to any third party (other than the Patient's eyecare Professional and his/her staff).

Registration and Contact Information

When a Patient registers to use the Services, the Patient will be required to provide certain information, including his/her name, address, email address, phone number and eye health related information. This information is used to ensure the relevance and appropriateness of information sent to Patients and to provide current contact information to the eyecare Professional.

Communication

EyeconX™ communicates to registered eyecare Professionals by providing them with updated eyecare content. It is the discretion of the eyecare Professional whether to disseminate the content to an EyeconX™ registered Patient(s). EyeconX™ also periodically contacts registered eyecare Professionals via telephone or email to obtain feedback on the use and functionality of EyeconX™.

There is absolutely no interaction between EyeconX™ and registered Patients unless Patients voluntarily ask support questions and/or provide feedback, via email, pertaining to the EyeconX™ application. Likewise, eyecare Professionals can contact EyeconX™ for technical support via e-mail, telephone or by clicking on the "live help" function provided on the EyeconX™ site.

Sharing of Information

EyeconX™ has no access to any Patient's personalized information. EyeconX™ does not share, sell, or rent Patient e-mail addresses to outside third parties. Anonymity is supported wherever possible. Disclosure of confidential information will occur only if required to do so by law. If there is such a requirement, EyeconX™ will endeavor to provide affected persons with an opportunity to challenge such disclosure through legal means.

Collection or Use of Aggregate Data

EyeconX™ will on occasion assemble aggregate data for use by interested third parties. This information includes non-personally identifiable data collected from individuals or groups of individuals regarding such things as demographic information, preferential information, and system usage information. This information will be processed so that no uniquely identifiable information regarding a particular individual can be interpreted or distributed to an outside party.

Retention of Data

EyeconX™ retains data collected from a Patient subscriber indefinitely provided that the Patient is a registered EyeconX™ user and the Patient's eyecare Professional does not erase his/her EyeconX™ record . If an eyecare Professional chooses to no longer subscribe to EyeconX™, Patient data is still available to be viewed by the Patient but CANNOT be updated by the eyecare Professional. Patients may request the removal of their personal information from the EyeconX™ system by contacting their eyecare Professional directly.

E-mail News / Personal Web Page

Upon registering with EyeconX™, Patients will be asked to provide certain information, such as his/her e-mail address, preferences for certain kinds of information and other forms of demographic details. This specific data will not be shared with third parties, but will be used to provide the Patient with customized information chosen by the Patient's eyecare Professional based upon the Patient's selected interests and preferences.

Passive Information Collection/Use of Cookies

As users navigate through a Website, certain information can be passively collected (that is, gathered without your actively providing the information) using various technologies and means, such as Internet Protocol addresses, cookies, Internet tags, and navigational data collection. We use Internet Protocol (IP) addresses on this site. An IP Address is a number assigned to your computer by your Internet service provider so you can access the Internet; it is generally considered to be non-personally identifiable information, because in most cases an IP address is dynamic (changing each time you connect to the Internet), rather than static (unique to a particular user's computer). We use an IP address to diagnose problems with our server, report aggregate information, determine the fastest route for your computer to use in connecting to our site and administer and improve the site.

EyeconX™ uses cookies to personalize each individual user's 'Meye Homepage' and to save users time when accessing the Services. A cookie is a small text file containing a unique identification number that is transferred from a Web site to the Patient's computer hard drive. Usage of a cookie is in no way linked to any personal or personally identifiable health information but is designed to recognize a user's authentication and personalize the EyeconX™ site. Cookies allow us to enhance the Patient's experience while using EyeconX™ through improved interactivity, customization of information, efficiency and download speeds to the user's computer. Cookies are considered standard practice for most sites operating on the web. A cookie cannot retrieve any other data from the hard drive, pass on a computer virus or capture a user's e-mail address. If a Patient declines cookies, he/she cannot view EyeconX™'s 'Meye Homepage' which includes personalized eyecare content For more information regarding accepting cookies, please refer to the browser's help menu.

We use a third-party service LiveSiteManager that uses cookies to track non-personally identifiable information about visitor sessions to enable live virtual communication with EyeconX™ site administrators.

Security

The EyeconX™ servers are housed in a well monitored and secure data centre where strict authentication is mandatory to access EyeconX™'s servers and related technology. Moreover, data protection mechanisms, security layers and data encryption measures have been implemented to prevent unauthorized access.

As part of the registration process, the Patient's account is password protected. EyeconX™ also encrypts the password within its database. It is important for the Patient to protect his/her password and computer from unauthorized access. When the Patient submits personal information to the secure areas of the EyeconX™ web site, we encrypt it using Secure Sockets Layer ("SSL"), a software encryption technology. Encryption is the industry standard means to protect information against unauthorized access and modification once it is stored in the database.

EyeconX™ takes reasonable steps to protect a Patient's personally identifiable information as the user transmits information from his/her computer to our site and to protect such information from loss, misuse, and unauthorized access, disclosure, alteration or destruction. Unfortunately, no data protection method or combination of methods can be guaranteed 100 percent secure. We strive to protect all personal information as described throughout this policy, but we cannot ensure or warrant our ability to do so. As a result, you use EyeconX™ content, products and services at your own risk. EyeconX™ will not be liable for disclosures of personal information due to errors in transmission or unauthorized acts of third parties. Moreover, where you use passwords, ID numbers or other special access features on this site, it is your responsibility to safeguard them. Passwords can be changed at any time, if you suspect your password security has been breached you must change your password immediately. The EyeconX™ System may contain links to third party sites. EyeconX™ is not responsible for the privacy practices or the content of such web sites. EyeconX™ recommends that you refer to each web site's Privacy Policy and Terms of Use for detailed information regarding their privacy practices.

Opting Out

EyeconX™ offers Patients the opportunity to remove themselves or "opt-out" of receiving information by email from EyeconX™ and the Patient's eyecare Professional in a way that is easy to understand. EyeconX™ offers the "opt-out" option in emails sent to the Patient by his/her eyecare Professional by way of the EyeconX™ system. EyeconX™ also offers Patients the ability to "opt-out" of receiving different types of information from their eyecare Professional during the registration section of his/her 'Meye Homepage'. Patients can modify their preferences within their 'Meye Profile' at any time.

Changes to this Privacy Policy

From time to time, EyeconX™ may change or update this Privacy Policy. EyeconX™ reserves the right to amend this policy in the future. If any substantial changes are made, we will make efforts to notify eyecare Professionals of such changes via email. By continuing to use EyeconX™ and receiving emails from your eyecare Professional the Patient agrees to be bound by any such revisions and therefore it is the responsibility of users to periodically visit this page of the web site and print the latest version of this Policy.

Patient Consent

By signing-in to EyeconX™ with a password and user name, the Patient consents to the collection and use of this information by EyeconX™ If there are changes to our privacy policy, we will post the revised Policy on this page so that you can always be aware of our Privacy Policy and practices, the information we collect, how we collect it, and the circumstances under which it may be disclosed.

Other Terms

If EyeconX™ experiences a change in ownership, we will make efforts to notify eyecare Professionals via email. By continuing to use EyeconX™ and receiving emails from your eyecare Professional, the Patient agrees to be bound by any such revisions and therefore it is the responsibility of users to periodically visit this page of the web site and print the latest version of this Policy.

The Patient's use of this site is subject to Terms of Use. The terms of this Privacy Policy of EyeconX™ are binding upon the successors and assigns of the business.

Information submitted to EyeconX™ shall be maintained in accordance with the terms of this Privacy Policy.
